Anonymous Feedback Form

The Puzzle Box Dance Studio’s goal is to create a safe, inclusive culture where everyone feels heard. Because of this, we have made available an Anonymous Complaint Form via Google Forms so that you can share your anonymous reports and feedback about culture issues, harassment (including sexual harassment), bias, or anything else that you’ve experienced or witnessed. 

 

While we encourage you to come forward to management when you’re able to, we know that sometimes it’s difficult to do that so this is an alternative way to share that feedback.

When a complaint is submitted through our Anonymous Complaint Form, we will do our best to follow the Reporting and Investigations Procedure found here. Please note that having something submitted through this channel can, in some cases, make it more challenging to complete a thorough investigation due to the anonymity component.
